  Request separate checks for each borrower on the loan.
• Sole Surviving Spouse Payment Reissue Request Form
  Request that full payment be reissued to a co-borrower who is the sole surviving spouse.
• Deceased Borrower Payment Reissue Request Form
  Request that a payment be reissued to an estate, heir(s) or next of kin.
• Payee Name Change Request Form
  Request that a payment be reissued with your name updated or corrected.
• Payment Disclaimer
  Notify the payment administrator that you do not want to accept a payment and do not want the payment reported to the Internal Revenue Service as potential income.
